Our doc was on call, patient came to ER with abdominal pain. She had delivered at home and had a uterine clot. Doc admitted her and performed a D&C. She ended up in ICU for several days with sepsis, endometritis, and pelvic thrombus. Can I bill for his admit with a modifier 57, surgery, and then for each day that he managed her care in ICU?

If the documentation reflects his services you can bill the admit and D&C, but the visits are included in the surgery global period.

If the postop visits are due to complications of the surgery, you may bill for those hospital visits with modifier 24 per ACOG.
