I am trying to gather information on what documentation is needed in order to bill Vein mapping with a CABG. If it can be billed with CABG. The following is the only documentation in operative report.
Procedure: 1. CABGx2 with LIMA-LAD, SVG- OM
2. Endoscopic vein harvest
3. Left atrial appendage clip
4. Sternal plating
5. Bilateral lower extremity venous mapping with duplex
Findings: Moderate quality greater saphenous vein
2. LV EF approximately 45 to 50% by intraoperative TEE at the end of the procedure
Procedure in detail: The patient is brought to the operating room and placed in the supine position. Bilateral lower extremity venous mapping is performed with ultrasound. General endotracheal intubation is performed without difficulty. Lines and monitors placed by anesthesia. Patient is prepped and drapped in the usual sterile fashion.
I could not locate a printed report with the outflows and inflows. Anesthesia did the documentation for the TEE. I greatly appreciate any advice , suggestions, or information that anyone could share with me to help answer my questions.
