If Path report reads: infiltrating adenocarcinoma, arising in background of high grade Barrett's esophagus, do you code the both the adenocarcinoma of esophagus and the Barrett's?

I would code both dx's as well since the carcinoma isn't causing the baretts esophagus, or vice versa.

Baretts

If we're still waiting on the baretts path, what code should be used to bill? Further, does a one time diagnosis work for future coding of baretts on follow up visits?
