For a sternal debridement, the anesthesiologist places and A-line and C-line in the holding area prior to surgery. Is it necessary for the anesthesiologist to dictate an op-note for the C-line? It is notated in PAU nurses note that it was placed w/location and w/out difficulty. The anesthesiologist also notated in the EMR Pre-Op Note that he planned to place the 2 lines.
 
There must be proof of the providers work. That proof is a note that includes: consent obtained, indications, prep/drape/use of local anesthetic, vein cannulated (for the central line), technique (Seldinger, etc.), any complications, signature.
