Is it appropriate to code removal and replacement of bilateral existing breast prosthesis with 19342 and 19328? Insurance would only cover 19342 as they considered the two codes as "mutually exclusive." Not sure how this can be as one code is for the removal (19328) and the other is the replacement (19342)
 
19342 is for Delayed insertion of breast prosthesis following mastopexy, mastectomy or in reconstruction. Doesn't that mean the initial implant has not been placed yet?
