My doc is a Physiatrist. (rehabilitation physicians, are nerve, muscle, and bone experts who treat injuries or illnesses that affect how you move.)


I'm looking for help with the CPT codes. We only bill the procedure.

Anyway he injected botox into the:

RT upper trapezius which I believe is 64646

RT cervical paraspinals might be 64633?

RT sternocleidomastoid might be 64616?

Okay. I figured out the codes. They are:

64646, 64616, 64616.

So I need help with the modifiers. Would it be coded as:
