Sorry guys but I have another coding issue that I am not familiar with.

The physician removed hardware from the distal tibia status post ORIF. He removed the hardware from the right ankle and tibia. After removing the hardware the doctor put in deminieralized bone matrix grafting to the right fibula.

Is this grafting billable? Can I bill hardware removal deep twice for removal of hardware from both locations?
