Physician describes as a bone biopsy, however the site of biopsy is a chronic ulcer to the bone and he did some debridement of soft tissue. I'm unsure whether this should be billed as debridement or bone biopsy---the ulcer was already down to bone. Which also leaves me wondering if this is coded as bone biospy, would I use a 52 reduced modifier on code on 20240-20245 since there was no anesthesia but the biopsy was open?

Please help...lost on what would be correct--op note attached:

Procedure: Bone biopsy

left foot prepped and draped, 5th metatarsal was exposed. Soft tissue was debrided off the bone. Rongeur was taken and under sterile conditions, small amounts of bone were removed. The was sent for gram stain..... Patient tolerated procedure.
