Wiki Blue Cross and TSH denials for age?

I'm noticing in the Michigan area BCBS is denying TSH testing with an age qualifier on it, as in the patient is outside of the acceptable age for a TSH test.

The small amount is stating to add up. Is there any documentation with BCBS concerning this? I work for a third party so I don't have direct access to provider bulletins for this lab charge.
