Confusing question but I have searched and searched and cant find a thing!! We have an orthopedic physician that leases space out of town from a chiropractor. We travel to that office once a week. The chiropractor has an xray machine. The chiropractor does not bill Medicare for xrays because Medicare will not cover xrays done by a chiropractor. We are wondering if our Orthopedic physician can bill the technical component to Medicare even though we do not own the equipment. Or......could the chiropractor bill Medicare the technical component and list our Ortho as the ordering and get it covered that way???? Our physician does not want the patient to have to pay cash for the tc portion of xray. I would appreciate any feedback! Thank you!!!!

When I worked for a family practice group we had an xray machine that we owned and we leased space to other specialists. The podiatrists that leased from us would charge global for the xrays and the family group would just charge the podiatrist a set fee per xray. Not sure if this was correct but maybe it will help.

Thanks for your response. Where it gets tricky in my mind is we (the specialist) do not own the equipment. Is it still ok for us to bill the technical component or do we literally have to own the equipment?

Technically the TC can only be billed by the person/entity that owns the equipment. I wouldn't do the global billing because if you get audited and they find out you dont own the machine then you're in trouble. I think your best bet is to try and get the chiro to bill with the ortho's name as the ordering provider and see if it can get paid that way. If not, then the patient may just have to pay. Definitely don't bill global though if you don't own the equipment.

Thanks

Aaron, thanks for your input. That is the answer I am leaning toward. I cant find any good documentation supporting that answer but I strongly feel that we should not bill the technical component. Thanks again!!
