We are trying to understand if we can charge patients for the premiere lens. I have found conflicting information so can someone assist me?

Do you mean multifocal and crystal lenses? If so, then yes, you may charge the patient for them. They are not covered by insurance.

Is there a problem if the physician purchases and brings the lens and then bills the patient from his end?

If the patient has Medicare the facility should provide the IOL. Medicares payment to the facility for the catract procedure includes a $150.00 allowance for a regular IOL. The facility may bill the patient the extra cost of the lens with a small markup ($25-$50).
