This is a question is in regards to a rehabilitation hospital and the recent denials for payment from commercial insurances. Has anyone else had denials from commecial insurances, stating that the rehabilitation code of v57.89 which has always to my understanding been the first code on the UB to describe the purpose or medical necessity of care performed at rehabilitation hospital, is no longer correct?

Per coding guidelines on page 12 item number 15 when the purpose for the visit/admission is rehab you assign the appropriate V57.x code as the first listed ex code. All V57.x codes are first listed only.
