Wiki Billing question, 3 visits in the same day

UHC has denied a claim, stating that the patient has had 3 visits, but it is on the same day of service. my question is if it is on the same day of service would it be just on the visits is it three seprate visits. Example patient had chemo, labs, office visit.

Is there a reason for the three separate visits? Is there a modifier which would qualify for this? Check your audit department to see as if I get it from a Clinical Standpoint:

Chemo visit would bring forth > Labs due to patient complaint which would discuss > Office Visit for Provider / Patient interaction.

Billing question

The patient comes in on the same day for all three services. But UHC stated that they are separate services, but why would they be if it is on the same day.
