When rendering services by the Physical Therapist as incident-to. How should the claim be submitted to the commercial payers. I read in one of Medicare updates that for these cases the PT must be an employee of the provider and the provider must be on the floor during the time the PT performs the service, medical documentation must indicate that the service rendered by the PT was supervised by the physician and documentation must also state the same. Should the doctor also sign off on these reports as well.
