What are the CA state rules regarding billing personal injury claims? Do the fees have to be the same as what we would bill to health insurances?

You're not going to find a rule that says you do or do not have to bill the same fee schedule. Here's the problem with different schedules... say the PI claim gets denied, and you bill Medicare for the services. Medicare notices you billed a different fee for the same service. Don't go asking for trouble, don't invite an audit, don't wave a red flag in a bullring.
Just use one fee schedule.
A side note: When we do legal work with attorneys on PI cases, we have to certify that the fees we are charging are our usual and customary fees. If we have a different fee schedule for PI, then that would not be true.
