Can anyone help me with billing NP/PA? I know Medicare's Incident To billing guidelines. What about other insurance(BCBS VA, Cigna, Aetna, Tricare, Optima)? Do we have to bill under the NP/PA's NPI or under the supervising MD(if they are on site)? Does the MD have to see the pt for the 1st visit and then the NP/PA can see them for any other visit? Thank you for your help!
 
Each individual payer has their own rules and it will depend on your contract and payer policies. There are some payers that do not credential mid-levels and they require you to bill them out under a supervising physician. It is in your best interest to make sure you understand their policies and what your contract reads to make sure you are compliant.

On another note our organization does not bill incident to they feel it is not worth the risk. If a midlevel can be credentialed we bill it under their NPI#.
