My biller is billing out the following codes and using modifier 59 on them. These are the codes for the panel we bill out on each of our samples.
I would like to know if we are doing this correctly because I am getting denials from the carriers because of unbundled and needing medical records and duplicates. Also I am being told that we should be billing out by the CMS billing and coding guidelines since we are laboratory.
