Codes billed separately J0897 (Prolia) admin 96372 & J9250 (Methotrexate ) admin 96401 (different dx codes). Coding error when billed together. Any suggestions on how to bill these codes together?
 
96372 bundles to 96401 (mainly because an injection of lidocaine as part of the IV start would be incidental) but if the 96372 is a distinct and separate service, as it is here, then you can append modifier 59 to indicate this.
