I have a situation I need help with. We have a doctor who has been seeing a patient and giving them Valstar. In the dictation the doctor did not put anything about 4 units, or 4 vials of medication. I know now that this is how the valstar is done. But shouldn't the doctor be putting something about how many unit's or vials were done?? You know the whole addage, if it's not dictated it didn't happen. All he put was that the valstar was mixed according to manufacturers instructions. I've gotten in a lot of trouble over this. Even though I wasn't the only person who billed it wrong, but that's a whole other issue If the dictation had said anything about 4 units, it would have been billed right the first time.
