Wiki Billing for urine dipstick done on outpatient setting by nursing staff

If a patient comes in after hours and lab is not in house: do you charge for the dipstick only and post to lab revenue center or do you use a low level outpatient E/M 99211 for the service and use the cost center as nursing or lab? I am having a hard time finding any documentation as to how to proceed. Thanks in advance for any guidance
