When a patient has an office visit to find out what, if any, vaccines are needed before traveling out of the country and that is the only reason for the visit, how should this be billed? Is there an ICD-9 code for this? Many times the patient does not even need any vaccines.

Many times its V65.49 however most carriers will not pay the exam or the immunizations related to travel. Make sure you get a signed waiver.:(

That code would work for diagnosis, but I don't think you can support an office visit charge since there's no chief complaint. This is really preventive care counseling, which wouldn't fall under the office visit codes.
