Wiki Billing for Temporary Physical Therapist

At an orthopedic practice we have physicians, therapists and Pta's.

We have a Physical Therapist out on FMLA, we have hired a temporary therapist, how should we bill the temporary therapist, should we bill incident to, to a physician on to another therapist in the office..???

Please help.
