Wiki Billing for team surgery

I have a surgery that took three surgeons and a total time of 28 hours. I am wondering if anyone has billed for a team surgery Mod 66 and has had success with it paying since it is a 'by report code". I usually bill co-surgeons and am wondering if the reimbursement rate is significantly higher for team surgery vs co surgeons. My payer is a commerical payer.
