If the physician is out of the office in surgery for example, does the PA bill for services rendered in the office under himself or under the physician?

Is it the same if the physician IS in the office?

Are any modifiers used or does the payor recognize the use of the PA by their NPI?

Depends on the payer. Some require it to billed directly (under the PA) and some require it under a MD. And there are no modifiers needed.

What if it is Medicare?

Medicare requires billing to be done under the PA's NPI, unless you are billing the visit Incident-to, but the requirements for Incident-to wouldn't be met if the MD is not in the office suite.
