We have a provider that owns her ultrasound equipement for MFM use. The Patient is inpatient at a hospital and a provider request a consult and a ultrasound by our provider.
we bring the patient over to the providers office and preform a consult and the ultrasound (we bill the hospital by contract for the ultrasound)
how would we bill for the E/M (consult)

1. which location to use?
2. which code (inpt or outpt)?

Why would your provider not see the patient in the hospital setting? I have never heard of a provider taking the patient from an inpatient setting to an office to be seen.

You must the POS of where the patient is registered. If this patient is a registered inpatient then your POS is 21 and you use regular office visit levels.
