Wiki Billing for implant that does not stay in a patient

Is it appropriate to bill and insurance carrier for an implant that was used in a case, but not left in a patient? Example - screw/anchor that was not the correct size or splintered the bone when inserted? Our vendor is charging the ASC for the implant as it can not be reused so the ASC is out the cost if it is not passed along to the carrier.

Any direction on this is welcomed.
 
The simple answer is yes – by billing with the appropriate modifiers, a hospital/facility/office may bill and be paid for procedures and equipment that are changed due to a patient’s condition or other unforeseen circumstances.
