Can you code/bill for a pap test (collection of specimen of diagnostic purposes) in addition to the E/M code during an outpatient Dr. visit?

If so, how do you distinguish that it is for diagnostic purposes and not screening?
Would that be reflected in the ICD-9 codes?

There is no CPT code for the collection of the PAP it is part of the E&M and the dx will be the reason for the encounter

I used to bill Q0091 and got paid.

Q0091 is a preventive not a diagnostic.
