if a patient comes in and labs are drawn in the office, but the patient's doctor is on vacation, who goes on the claim? The NP drawing the blood or the dr because they ordered it?

I am saying the NP because the Dr is out of office, but the office is saying Dr ordered it Dr bills it
 
The NPI in field 24j is for rendering/supervising provider. If the provider is not present in the office he is neither rendering nor supervising. Use the NP NPI since that was the rendering person.

I am trying to find documentation that clearly states that the labs must be billed under a provider who is present in the office since clinical lab services and venipunctures are not subject to incident-to guidelines.

can anyone point me in the right direction?
