We have several physicians on the family practice side that are performing this procedure. We are looking to see if there are any documention requirements that need to be in the note in order to be able to bill for these. I'm including what we are commonly seeing in the notes, as I don't feel this is enough but want to make sure before moving forward with some education on this. The verbaige we see most often is this " Anoscopy: Rectal tissue is pink and moist." that is the extent of the procedure note and they inclue this in the Exam portion of the note. Any help/advice/direction that can be given on this would be greatly appreciated.
