Billing Z23 for the Flu or Covid Vaccine; can it be billed if I enter the Dx code on the claim though the Dx is not on the note just that it was administered. I did let them know that the DX code needs to be included in the note. Please advise. Can anyone forward me a link in where the Dx code needs to be on the note.

Vaccines are usually given by clinical staff who do not diagnose the patient themselves, so the diagnosis would be on the order that was written by the physician, not on the vaccination note itself. Since these are almost always a preventive service, it's usually going to be self-evident that Z23 is the appropriate diagnosis, but technically speaking the provider who orders the service should be documenting the reason for it in the patient's medical record or on their written order. Sometimes vaccination may be given under a 'standing order', in which case there wouldn't be a specific diagnosis given, and in that situation I think that Z23 would be appropriately assigned.
