I am looking to create an evaluation sheet for the billing department. I was wondering if anyone had a template that I could use? I have a generic evaluation form but need one more directed to the billing department. I have looked through the forum and see this was asked back in 2015 but do not see any thread, answer or documents attached to that question. If anyone is willing to share one you already have I would appreciate it. Thank you.
