Is there anyone out there that is or has billed 99490 chronic care management or 99495 transitional care management services. I work for cardiology with a high percentage of patients that require additional and time consuming care with our nurses even after being seen by the doctor. I would love some insight as we are looking to use these codes, such as reimbursement and the type of documentation required. Thanks
