Wiki Billing E&M codes with MIPS codes

Is anyone having denial from Anthem BCBS when an E&M code is billed with a MIPS code? We've started seeing this denied in the past week or so stating that the codes can't be billed together. I've called Anthem and all the rep can say is to appeal.
