I am trying to bill for ASC services and can't figure out what form to use the HCPC 1500 vs UB04. We have some insurances that will accept the UB04 but Medicare denied all our claims and wants them resubmitted on the 1500 form. Now I have a few SR plans that are also requesting the 1500 form and not the UB04. Is there a rule regarding when to use each form?

The majority of the carrier will want you to use the UB, Medicare wants the HCFA as well as some workers comp carriers. There are no rules, its hit or miss.

HCFA vs UB04

We are in Iowa. Medicare and Medicaid want their claims on the HCFA form, all the other carriers want the UB04. I made sure I was asking them what they wanted us to bill on as I was contracting with them.
