Wiki Billing an E/M with x-ray when you own equipment and bill global for the x-ray.

I know that if you own the x-ray equipment, you don't use the x-ray towards medical decision making. If all you do is an x-ray with a repeat injection, do you still bill an E/M, and if so, what do you base the medical decision making on?

Example - patient is seen for recurring finger joint pain, x-ray is done, and doctor decides to just do a repeat injection for CMC joint osteoarthritis. Normally I would only bill the injection, if all we did was a repeat injection for an established issue, so do I only bill the injection and the x-ray?

Is there an office visit documented by the provider that would support billing an E/M with a modifier 25? If not, I would only bill the injection and x-ray.
