Wiki billing 99211

I was wondering if anyone could help me find information on billing 99211. What kind of documentation will need to be in place. For Example, do we need to include vital signs with every 99211. Please advise me where I can go look for this information.

99211

This is covered under module 3 of our AAPC coursework.

99211 can be billed by a NPP as long as the provider is present in the office and has reviewed and signed off the NPP's findings.It still requires a CC, HPI, exam, diagnosis and plan but minimal requirements. Often referred to as 'incident to' visit. Vitals are not always required, unless pertinent to visit.
