Wiki billing 2 amputations

If partial 4th and 5th ray amputations are done, but then during the procedure the doctor notices that the osteomyelitis is more extensive than previously thought, and ends up doing a transmetatarsal amputation, could the toe amputations be billed in addition to the transmetatarsal amputation?
Oddly, toe amputations do not bundle with the transmetatarsal amputation.

Any thoughts, please?
