If both CPT code 76872 & 55700 were rendered in the OR and both codes were billed under Revenue Code 360, but Noridian rejected the claim due to CPT 76872 was billed under RevCode 360. There was no imaging record and documentation from imaging dept. CMS does not provide explicit instructions; therefore, revenue code assigned is where the service is performed; in this case in the OR revenue code 0360. How do you bill in this case? Is there a way this edit can be bypass? Any insights or guidelines is appreciated.
