Wiki Bilateral EMG w/Nerve Conduction, Help with getting paid

We are having issues (just recently) getting bilateral EMG's paid. My doctor did a 95886 bilateral upper with nerve conduction. We have billed it as 95886 Rt, LT, 95886 x2, 95886 with a 50 modifer, all have been denied. Any suggestions?

We've been billing with just 95886 x 2/3/4, and haven't had any issues that I'm aware of. If you've tried that way, and the others you listed, my only other recommendation is modifier 59 or XS, whichever the payer allows.
