We are having a BCBS issue. Chiro office here, we bill manual therapy and adjustments. We have re-diagnosed and rebilled, to no avail. The CPT codes are 97140 (& POINTER), 98941, 98943, 97110, 97012 with ICD-10 codes M99.01, M50.121, M99.03, M51.17, M99.02, M51.35, M99.06, M25.561, M99.07, M25.511. We don't necessarily bill 97140 and 98941 on the same day, but we have, and we did attach a mod -XS to the 97140, and sporadic claims are denied! We did not have any problems with this until April 2022, we have called BCBS on many occasions and they will only say that a dx code is the problem. They are all valid codes and CMS approved. Does anyone have insight or advice? What are we doing wrong??
