What are the documentation requirements for an annual wellness visit (AWV) when conducted via telehealth, mainly telephone only? My providers feel that they just need to d
it was done but I disagree. The AWV form is sent to patient to complete, then provider contacts patient via telephone and reviews with them. I do not think this meets
the Medicare requirements, something more needs to be documented in chart other than AWV was done. Any advice?
