Wiki AWV in the Home Setting

Does anyone know if an AWV can be performed in the Home setting and if so can it be done by an extender and I know it would be billed under their provider number and paid at 85%.

Note sure, But do you a template for the AWV and IPPE, I looking for one that has all the elements(checklist). THANKS
