Shall we code this as: 96413 - avastin, 96417 - Leucovorin or Leucovorin still be coded as 96367? Please help
 
What administration have you been using when Leucovorin is being administered concurrently with another drug since this change using chemo admins? I have run into issues and I wanted to reach out and see what others are doing.
