Optum on behalf of Healthfirst is denying claim. Per medical record attending physician is different than billing provider.
Attending physician dictated op report. billing provider is listed as provider on op report who serviced patient

Has anyone experienced issued?

Why is there a difference in providers? Are you reporting appropriate modifiers that help tell that story?
Was the attending physician a participant in the procedure? If not, why did s/he dictate the op report instead of the performing provider?
