This impatient patient had an angiogram two days prior. Decision was made to bring patient back down for an attempted Peripheral intervention. However after an approx. one hour the physician was unable to cannulate the popliteal artery and aborted the procedure and consulted surgeon to evaluate for possible bypass graft. Patient was returned to the floor.

There is insufficient documentation to code U/S guidance therefore no code there was no fluoro used. Can I use modifier -74 (versed administered) with CPT 37224? or can I not code for any type of procedure.

There were very little supplies used. Two sheaths, one wire, angio pack, surgery prep, leads ect....


Questions: Do you think this is a procedure that would qualify for 37224-74?

This is to code for the cath lab not for the physican..

PROCEDURE PERFORMED: Attempted angioplasty of the left superficial
femoral artery.
INDICATION: Total occlusion of the left superficial femoral artery and
nonhealing wound of the left foot.
TECHNIQUE: The patient was described the procedure, benefits and
complications and informed consent was obtained. The patient was placed
in the prone position on the angiography table. The left popliteal fossa
was surgically prepared and draped. Attempts were made to cannulate the
left popliteal artery using ultrasound and Doppler guidance; however,
the artery could not be cannulated. At that time the procedure was
stopped. The patient tolerated the procedure well.

I would code CPT 37224-73 and all supplies used. It does not state in the report that anesthesia was used?
