One of my physicians completed a level L4-5 ATP with posterior spine fusion.
Small portion of the op note:
Thorough discectomy was performed at L4-5. At this time appropriate size cage was packed with allograft and then placed into the disc space. A lateral plate was placed on the lateral aspect of L5 and appropriate sized screws were placed at L5. With the patient still in lateral decubitus position we began placing percutaneous screws.... there is more of the op report but it only speaks about placing screws.
Would you bill 22558 and 22612? How would you bill the fusion portion?
