Wiki Asthma with allergies

Does the documentation have to state asthma with allergic rhinitis to use the combo code 493.0X? My doctors frequently list asthma and allergic rhinitits but do not associate the 2 and I am just not sure if I should be coding them separately or if the association is assumed.
