Wiki Assisted living E&M codes

I work in family practice where my doctors see patients at assisted living facilities. If the patient comes into the facility clinic to be seen, do we bill a regular office E&M (99212-99215) with place of service 11 or do the assisted living E&M's apply with POS 13?

The place of service would be 13 if the doctor physically goes to the facility to see the patient.
