Wiki Arthroscopic notch plasty of the right internal condylar of femur plus micro fracture

PRE-OPERATIVE DIAGNOSIS: Impingement right reconstructed ACL**+ Grade 4
medial femoral condyle ulcer.

POST-OPERATIVE DIAGNOSIS: Impingement right reconstructed ACL**+ Grade 4
medial femoral condyle ulcer.

OPERATION PERFORMED: Arthroscopic notch plasty of the right internal condylar of femur plus micro fracture of medial femoral condyle

Under general anesthesia and tourniquet control, arthroscopy was done in the usual anterolateral and anteromedial portal. There was a chondral lesion grade IV ulcer in the medial femoral condyle measuring 5 mm x 2 cm. Microfracture technique was done. Then, the ACL was intact there was severe impingement on it. So, arthroscopic notch plasty was done. The lateral meniscus was found to be intact. The patella was good. Wound debridement was done. Insertion of drain and wound closure.

Can we code 29879 for this? or any other code we have add? Thanks

Yes on 29879 even though I'd like to see that "microfracture with awl (tool) was done down to bleeding bone" but most documentation is not perfect. The notchplasty (29999) would not get paid as it's in same area as 29879.
