Op note states
L3-4, L4-5, L5-S1 posterolateral fusion

and

L5-S1 and L3- L4 Interbody fusion


How would you code this? I came up with 22633 L3-4, 22614 L4-5 and 22634 L5-S1. Is this correct?

I would bill 22633 L3-4, 22612 59 L4-5 and 22634 L5-S1. I have never been able to get paid billing 22614 without a 22612 even on appeal when I've explained 22633 is the primary code. The insurance companies will pay 22612 59 though. 59 is valid for the procedure because it is at a different level.
